Elevated Medical Cannabis was created to help Kentucky patients better understand and navigate the medical cannabis certification process. For people living with chronic medical conditions, medical cannabis may offer another tool to help manage symptoms and improve quality of life.
Medical cannabis deserves to be approached thoughtfully and treated with respect as a medical treatment option. For patients interested in exploring medical cannabis, the process can feel confusing—especially with a new program, changing regulations, and so much information available.
At Elevated Medical Cannabis, the goal is to provide thorough medical evaluations, determine whether patients meet Kentucky's requirements for certification, and help patients understand the certification process and important safety considerations surrounding medical cannabis.
EMC also hopes to be part of the changing conversation around cannabis by reducing stigma and providing reliable, medically focused education.

Steps to Get Your Kentucky Medical Cannabis Card
1) Schedule Your Evaluation
Schedule an in-person evaluation with Elevated Medical Cannabis to determine if you qualify for a Kentucky medical cannabis certification.
2) Complete Your Certification Visit
Your medical history and qualifying condition will be reviewed. If eligible, your written certification will be submitted to the Kentucky Office of Medical Cannabis.
3) Apply for Your Medical Cannabis Card
After certification, apply through the Kentucky Office of Medical Cannabis patient portal and pay the $25 state application fee.
4) Receive Your Medical Cannabis Card
Once your application is approved, the Kentucky Office of Medical Cannabis will issue your medical cannabis card.
5) Visit a Licensed Dispensary
Once your card is issued, visit a licensed Kentucky medical cannabis dispensary. Bring your medical cannabis card and a valid government-issued photo ID to purchase medical cannabis.

Executive Order 2026-318, issued June 2, 2026, clarified Kentucky’s definition of a “qualifying medical condition” to include additional conditions.

What to Bring to Your Appointment
Please bring a valid government-issued photo ID and medical records that document the condition for which you are seeking medical cannabis certification.
Examples of helpful records include:

A current medication list
For chronic conditions, records should support that the condition is ongoing and not solely related to a recent or isolated acute injury or illness.
Initial Certification Evaluation

In-person medical evaluation for patients seeking Kentucky medical cannabis certification.
The initial evaluation must be completed in person.
Annual Renewal Evaluation
$100
Annual evaluation for established patients renewing their medical cannabis certification.
Renewal visits may be completed by telehealth when appropriate.

Kentucky State Application Fee
$25
Application fee paid directly to the Kentucky Office of Medical Cannabis when applying for your medical cannabis card.
This fee is separate from your certification evaluation fee.

When You Arrive
Elevated Medical Cannabis is located on the lower level of Four Rivers Internal Medicine & Quick Care.
When you arrive, enter through the main entrance and let the front desk know you are here for an Elevated Medical Cannabis appointment. You will be directed to the elevator to the lower level. Once downstairs, have a seat in the lobby and a member of our team will meet you and escort you to an exam room.
